The Gemora in Makos 10B quotes a famous Midrash which speaks of two murderers - one who has killed with premeditation and the other who has killed accidently. There were no witnesses to either death. Thus death by Bais Din for the one who murdered intentionally or sending into exile, to the Arei Miklat, for the one who murdered accidently, could not be implemented. Hashem arranges that both murderers meet at a hotel. In front of witnesses the one who previously murdered accidently falls onto the intentional murderer and kills him - again accidently. The intentional murderer has met the fate he deserves. The accidental murderer now has witnesses who can testify that he killed B’shogeg and he can be sentenced to Golus – exile.
